Our data science initiatives transform patient care, in part, through predictive models that help identify at-risk patients earlier and optimize treatment strategies. This can help our Registered Nurse Health Coaches deliver personalized care with greater efficiency.
Registered Nurse Health Coach connection
Our member outreach and support is targeted, in part, based on the results of our data analysis that identifies members who are shown to be in need of care beyond what they are currently receiving. This often includes a call from one of our Registered Nurse Health Coaches, which can have a tremendous impact on members’ health. A Registered Nurse Health Coach interaction can assist a member in:
- Coping with a serious illness or accident, like cancer or stroke
- Reviewing a doctor’s discharge instructions
- Coordinating services related to a hospitalization, procedure, or illness
- Understanding medications and why and how to take them
